Experience with directing treatment utilizing aides or technicians. Previous management experience preferred but not required. Strong time management and multi-tasking skills. The Physical Therapist is a key associate at Hope Network whose primary responsibilities include: Provides physical therapy services as prescribed by consumer’s physician for adults and adolescents with head injuries, spinal cord injuries, chronic pain, general orthopedic rehabilitation needs, and/or neurological behavioral issues. Work with supervision according to Section 333.16109 of the Michigan Public Health Code ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: 1. Regular and predictable attendance is an essential requirement of this position. 2. Conduct evaluations on consumers as requested by prescribing physicians. 3. Provide physical therapy services which may include: conducting evaluations and assessments, improving strength, balance, endurance, coordination, joint range of motion, muscle flexibility, movement patterns, gait of clients, designing and instructing in execution of, home exercise programs to client and direct care staff, prepare clinical treatment plans and progress notes for clients seen, arranging appropriate external supportive devices and mobility related equipment. 4. Provide regular trainings for direct care staff on general consumer transfers and gait exercises. 5. Consult with both direct care staff and professional team members, especially PTA’s and Aides, regarding treatment and consumer progress. 6. Conduct trainings, in-services, and participate in team meetings as assigned. 7. Participate in the development and implementation of the program TQO/Quality Improvement plan. 8. Monitor equipment in physical therapy department for needed repair or replacement. 9. Serve as a member of an interdisciplinary team for assigned clients. Participate in client goal development, coordination of services, and discharge planning/family education as needed. 10. Attend and verbally present at team meetings, clinical reviews and in-services. 11. Communicate clearly to team members and interested parties on the Progress, functional status, recommendations and needs of the consumer. 12. Familiarity with all departmental equipment, and ability to implement equipment according to consumer goals/needs. 13. Schedule clients weekly as prescribed and report to supervising therapist any changes in frequency, status and improvements as needed. 14. Participate in the orientation and training of new staff, student interns and volunteers as assigned by the supervisor. 15. Other duties as assigned. With treatment centers based in Farmington/Farmington Hills, this position serves participants in Oakland County and the surrounding communities. This autonomous and professional role includes: providing PT assessments and interventions to participants, clinical collaboration with a comprehensive interdisciplinary team, and opportunity for community based or home based interventions as warranted. This position offers flexible scheduling to meet the needs of the PT and program together. Deliver rehabilitation services for those who have sustained a TBI, SCI, orthopedic injury and/or other neurologic compromise. Assess program participants through appropriate evaluation procedures and develop written treatment plans for each. Administer appropriate treatment programs through individualized treatment, group therapy, or co-treatments. Communicate program participant needs to appropriate staff, including HEPs and DME recommendations, and provide in-services and training for staff. Provide needed re-evaluation and programmatic changes as necessary. Maintain timely and appropriate administrative and department documentation. Ensure adherence to accreditation standards, licensure standards, professional code of ethics, and confidentiality. Attend programmatic meetings as requested for team reports, policy setting, and exchange of information; attend other appropriate meetings/conferences as needed. Attend clinical meetings to exchange, discuss, and evaluate participant treatment programs. Serve on rehabilitation treatment team(s), communicating with physicians, staff, professionals/paraprofessionals, consultants, families, and community resources to ensure effectiveness of physical therapy program. Supervise PTAs per delegation protocols and review documentation. Offer peer review of Utilization Management. This position requires workers to comply with the Centers for Medicare & Medicaid Services (CMS) Ruling. Workers in certain health care setting must either (1) obtain the primary series of a COVID-19 vaccination or (2) seek and receive approval for a medical or religious exemption.
