Athletico has facilities throughout Arizona, Illinois, Indiana, Iowa, Kansas, Michigan, Missouri, Nebraska, Ohio, South Dakota, Texas and Wisconsin, and offers more than 40 specialty rehabilitation, outreach, and fitness services. Position Summary At Athletico, the Physical Therapist’s role is to evaluate, design and implement a patient’s plan of care. Each Plan of Care is created to increase, maintain or restore function, minimize or prevent disability, and assist patients in reaching their maximum performance levels. Essential Duties and Responsibilities • Providing interpretation of prescriptions, performing an initial evaluation, and establishing a treatment plan with functional and/or work related goals • Appropriately progressing the treatment and goals • Periodic and timely written communication to physicians • Appropriate discharge management and patient follow-up • Supervision of rehabilitation aides and administrative staff as needed • Supervision of PTAs when applicable per the appropriate state practice act • Responsible for documenting all treatment and completing all written communications in a timely manner, including but not limited to, initial evaluations, progress notes, discharge summaries, daily S.O.A.P. notes, functional status reports, functional capacity evaluations and insurance/plan of care forms as necessary • Establishing and developing professional relationshipswith Athletico’s referral sources including, but not limited to, physicians, PAs, ARNPs, case managers, etc. • Community Services (such as): attending health related fairs, presenting on orthopedic injuries or prevention, providing coverage at special events such as sport camps, races, tournaments & lectures, etc. • Informing physicians of our services, locations, quality patient care and philosophy to be the best provider • Accompanying patients to their physician visits (patient visits) Share Your Career Goals / Send Your Resume College Relations 625 Enterprise Dr Oak Brook IL 60523 Careers@athletico.com

Demonstrates the interpersonal skills needed to communicate in a tactful, sincere, effective manner with patients, families, healthcare professionals and various Hospital staff. Must maintain current BLS certification. Working Conditions Works in a patient care environment requiring physical exertion, frequent changes in job demands, and certain undesirable patient care activities. May be exposed to bio-hazards and normal hazards encountered in a community environment. Physical Demands Requires the physical ability, stamina, visual acuity, and manual dexterity (i.e. to walk/stand for prolonged periods of time, push carts/wheelchairs up to 50 pounds, to position/lift patients at a maximum of 35 pounds unassisted, over 35 pounds requires assistance, provide CPR, etc.) to perform the essential functions of the position.

There is job security, full benefits, retirement, vacation, and sick time. Extremely low turnover, longevity here. Position open due to growth COMPENSATION: • ~$39/Hour for New Grads (~$81,000 annually) – Range to $58/hour (~$121,000) • Plus, ~$20,000 in additional Bonuses to help pay school loans and for sign on bonus • Annual allowance for Continuing Education, plus continuing ed brought on campus • Mileage reimbursement (~ 60 cents per mile) • Career advancement opportunities within this large healthcare campus MENTORSHIP PROGRAM: New grads have done very well here! • Paired with another Physical Therapist for 5 weeks or more • Taught the visits, charting, policies and procedures • Next, meet with an Educator for training of the computer system • Then split a schedule with the Physical Therapist • Eventually on your own and have the therapist as a backup to ride along OVERVIEW: • Schedule: M-F flexible hours / 40 hours per week / paid at time and ½ for any overtime • Given a Laptop, nursing bag, BP cuff, pulse oximeter, and stethoscope • Join a large Home Health Team: 9 PTs, 3 PTAs, 3 OTs, and 2 PRN OTs • On average, productivity expectation is about 5 • A typical day is 3-5 patients / average miles per day is 40-60 miles • EMR: Delta Health Technologies product Crescendo • Patients are those discharged from Acute Care Hospital and Inpatient Rehab Hospital • Ortho and Neuro Adult patients / generalized weakness, COPD, CHD, Orthopedic patients, Stroke, some TBI and SCI, plus other LOCATION / West U.S. / a cost of living which is equivalent to the Midwest States! A large healthcare campus in an outdoor recreational area; mountain biking and hiking, rock climbing, white water rafting, kayaking, water skiing, snow skiing, fishing, canoeing, cycling. It’s a small town feel with amenities of a larger city. Surrounding scenery is breathtaking, and downtown is thriving with arts and dining. A lot of fresh produce, farmers markets. New restaurants, wine tasting, art galleries, festivals, unique shops downtown. Day trip to beaches. Requires relocation out of Portland, OR.
